Job summary

HarborCHC is seeking an Enrollment Coordinator to assist patients in enrolling and maintaining health insurance subsidies, anchoring them to Harbor Community Health Centers. The role requires bilingual English/Spanish communication and strong data/documentation skills.

Responsibilities include completing eligibility assessments, submitting applications, following up on status, and guiding patients to suitable plans.

Qualifications

• High school diploma or equivalent.
• Fluent in English and Spanish.
• At least 2 years of healthcare customer service experience in the target community.
• Current Basic Life Support (BLS) certification.
• Experience at a Federally Qualified Health Center preferred.

Responsibilities

• Assess eligibility for health programs and subsidies (e.g., Covered CA, Medi-Cal, Medicare).
• Promote HarborCHC services to the community as a medical home.
• Assist patients with applications for programs they qualify for.
• Submit enrollment information to program platforms like Covered CA.
• Conduct follow ups to verify enrollment status and resolve issues.
• Help patients select health plans based on needs via program platforms.
• Maintain HarborCHC enrollment documentation in ECW and daily logs.
• Coordinate with staff to assign patients to HarborCHC and IHA appointments.
