Job summary

Sea Mar Community Health Centers is seeking a Community Resource Navigator to educate eligible individuals about health coverage options and assist with applications. The role involves outreach, referral networking, and helping high-risk patients schedule initial appointments for medical, dental, and care management services.

The position requires bilingual English/Spanish, strong customer service, and the ability to travel across assigned counties.

Qualifications

• Excellent customer service skills and professional demeanor.
• Bi lingual English and Spanish preferred with strong reading/writing.
• AA or BA in Social Services or Business Administration with relevant experience.

Responsibilities

• Plan and conduct outreach to raise awareness of health coverage enrollment opportunities.
• Develop and distribute informational materials and establish referral relationships.
• Assist uninsured individuals with health coverage applications and navigate options.
• Serve as a navigator to establish Sea Mar as the medical home for consumers.
• Schedule care manager appointments and coordinate access to services.
• Maintain daily records and support quality improvement within Managed Care.
