Job summary

The Staff Pad is partnering with a Federally Qualified Health Center in Pomona, CA to recruit a Bilingual ECM Behavioral Health Care Manager. This full-time, non-exempt role supports members through care coordination, scheduling, and connections to healthcare and community resources.

Travel between clinics may be required. You will work with an interdisciplinary team to improve access to care, address social determinants of health, and ensure accurate documentation while adhering to IEHP and UDS

Qualifications

• High school diploma or equivalent required; degree preferred.
• LCSW/LMFT experience required.
• 3+ years of relevant experience preferred.
• Experience in care coordination, community health, case management, or social services required.

Responsibilities

• Manage an assigned ECM member caseload in accordance with Medi-Cal guidelines.
• Conduct outreach to connect eligible members with ECM services and assist in scaled metrics.
• Conduct assessments and surveys to identify member needs, risks, and care gaps.
• Track required assessments and Shared Care Plans; document interactions accurately in the EHR.
• Coordinate appointments, transportation, referrals, and follow-up care.
• Help members navigate healthcare, behavioral health, social services, and community resources.
• Communicate with the care team to support coordinated care and referrals.
