University Health is seeking a community health outreach professional to coordinate outreach activities and assist with CMS grant applications. The role provides information, education, and enrollment assistance for Medicaid, CHIP and CareLink, acting as a liaison with community partners and internal teams.

Responsibilities include engaging at school districts and events, delivering educational presentations, and collaborating with other programs to identify grant opportunities.

Qualifications

• Bachelor’s degree required; health-related field preferred.
• Degree in health education or social science preferred.
• Two years’ experience in community health promotion or preventive programs preferred.
• Experience delivering public or educational presentations with strong communication skills required.
• English and Spanish fluency required.

Responsabilités

• Coordinates community outreach and assists with CMS grant applications.
• Provides information, education, and enrollment assistance for Medicaid, CHIP and CareLink.
• Serves as liaison between internal and external customers across the Health System and community.
• Supports outreach and education at school district and community events.
• Prepares and conducts educational presentations on the grant and programs.
• Collaborates with other Health System programs and partners to identify grant opportunities.
