Job summary

AACI in San Jose, CA seeks a Patient Engagement and Enrollment Coordinator to lead community outreach and advocate for health coverage access. The role focuses on promoting AACI services and enrolling eligible individuals into Medi-Cal and other plans.

Key duties include outreach planning, screening uninsured families, coordinating with partners, and building referral networks. Strong communication and public engagement are essential for success.

Qualifications

• High School diploma or equivalent.
• Two or more years of experience with insurance enrollment navigation, community outreach, public health, or community relations.
• Healthcare setting experience as a Certified Enrollment Counselor strongly preferred.
• Experience facilitating public meetings/public forums is a plus.

Responsibilities

• Develop and implement a community outreach plan to promote AACI services.
• Inform and screen uninsured families eligible for Medi-Cal.
• Collaborate with local organizations to promote insurance options and enrollments.
• Develop outreach materials, presentations, and distribute information to patients and partners.
• Engage prospective patients and enroll eligible individuals into appropriate insurance plans.
• Reach out to patients to address gaps in preventive and ongoing care.
• Build relationships with community influencers to increase AACI visibility.
• Participate in media events and complete other duties as assigned.
