Job summary

Upward Health in Rio Rancho, NM seeks a Community Outreach Specialist to educate potential patients, engage them in conversations, and enroll them into programs. You will manage a personal caseload via phone outreach and occasional in-person visits, coordinating with the Outreach Manager for program success.

You will work in a fast-paced, mission-driven setting, using diverse outreach methods, and leveraging strong communication to build trust and guide patients toward care.

Qualifications

• Excellent verbal communication and ability to persuade and educate patients.
• Strong interpersonal skills to build trust and rapport quickly.
• Ability to manage a personal caseload with organization and efficiency.
• Self-motivated, able to work independently toward outreach goals.
• Comfort with fast-paced environments and varied outreach methods.
• Proficient in documenting and using patient information in systems.
• Fluency in English; Spanish proficiency is a plus.

Responsibilities

• Educate potential patients about Upward Health offerings and enrollment steps.
• Manage a personal caseload, prioritizing outreach goals and follow-ups.
• Conduct phone outreach and in-person visits as needed.
• Coordinate with Outreach Manager and team to optimize engagement.
• Document interactions accurately in the company systems.
