Job summary

The Community Clinic of Maui, Inc. is seeking a Community Health Worker to support patients with medical and social needs within the PCMH model. You will assist with care coordination, education, navigation, and follow-up to improve health outcomes.

Responsibilities include outreach in Maui communities, helping with appointment scheduling, translating and interpreting services, and linking patients to resources. A high school diploma and bilingual English/Spanish proficiency are preferred.

Qualifications

• High School Diploma or equivalent.
• Experience in a multi-cultural/community-based setting preferred.
• Bilingual in English and Spanish; other languages desirable.
• Excellent written and spoken communication.
• Ability to establish positive relationships with staff and community partners.
• Basic computer skills.

Responsibilities

• Care coordination and navigation for patients.
• Resource identification and referrals.
• Education and communication in multiple languages.
• Assist with appointment scheduling and follow-up.
• Provide psychosocial support and translation services.
• Coordinate transportation when needed.
