PMCH is seeking a Community Health Worker to serve as a trusted liaison between the organization and the community. You will help individuals access health services, provide education, and support overall well-being.

The role emphasizes outreach, advocacy, and culturally responsive support, including home visits and connecting clients to community-based services. You will work with CHW supervisors and local hubs, manage caseloads, use screening tools, and refer clients to prenatal and family

Qualifications

• High School Diploma required; Associate’s degree preferred.
• 1+ years of experience in community outreach or social services required.
• Bilingual fluency in English and Spanish required.
• Experience with outreach to Bergen and Passaic Counties health and social services preferred.
• Excellent verbal, written and interpersonal communication skills.

Responsabilités

• Serve as liaison between PMCH and the community to improve access to health services.
• Conduct home visits and health education sessions; assist with care coordination.
• Refer clients to health insurance, prenatal care, WIC, substance abuse, mental health, and other services.
• Document case management, assessments, referrals, and follow-ups in program database.
• Engage in community outreach and partnerships to address social determinants of health.
