Job summary

Community Clinic of Maui is seeking a Community Health Worker to support patient recruitment, education, navigation, and care coordination in Maui’s diverse communities. The CHW will focus on social determinants of health and direct medical services under the PCMH model.

Responsibilities include language-access services, translation support, and helping patients access programs, transportation, and essential resources.

Qualifications

• High school diploma or equivalent required.
• Written and oral fluency in English and Spanish required.
• Experience in a multi-cultural/community-based setting preferred.
• Basic computer skills and knowledge of medical terminology a plus.

Responsibilities

• Perform care coordination and patient recruitment within Maui community.
• Provide education and navigation to patients, with language support.
• Assist with appointment scheduling and transportation coordination.
• Support chronic disease management and care coordination under PCMH model.
• Deliver culturally appropriate services in multiple languages.
