Qualifications

• Valid driver’s license and driving record meeting organizational requirements.

Responsibilities

• Conducts community outreach via home visits, events, and partnerships.
• Provides culturally and linguistically appropriate health education.
• Coordinates care and facilitates referrals to clinical services and social supports.
• Offers coaching on nutrition, chronic disease management, and preventive health practices.
• Advocates for access to healthcare, housing, and other social determinants of health.
