Qualifikationen

• High School Diploma or GED required.
• 1 year of Community Health Worker experience or related field.
• Lived experience in the community served.
• Certified Community Health Worker (CCHW) within 12 months of start date.
• Bilingual Spanish & English required.

• Initiate outreach efforts related to preventive health and care gaps.
• Educate members on preventive health services and referrals.
• Coordinate access to services and community resources.
