Job summary

Henry J Austin Health Center, Inc. seeks a Community Health Worker to provide emotional, educational and social support for clients facing barriers to care. This on-site, bilingual role engages with diverse communities, helps schedule appointments, coordinates transport, and connects clients with housing, insurance and other services.

You will educate about treatment, document outcomes, track referrals, and collaborate with medical teams to improve access and retention in care.

Qualifications

• High school diploma or equivalent required; college degree a plus.
• One year of related experience working with at-risk populations and performing outreach.

Responsibilities

• Engages in case finding and community outreach to bring out-of-care patients into care services.
• Accompanies clients to medical, mental health, or social service appointments to improve attendance.
• Educates clients and supports adherence to care plans, including obtaining necessary documents and housing/insurance info.
• Maintains timely chart documentation and data for quality improvement.
• Follows universal precautions and participates in agency COOP plan.
