Job summary

Sea Mar Community Health Centers seeks a Community Health Worker to connect clients with health and social services in home, community, and office settings. You will document interventions, participate in Pathways training, and work with HUB partners to support clients in achieving health goals.

You will serve diverse communities with a focus on Latinos in Washington State, using motivational interviewing and goal setting to improve care coordination and access to services.

Qualifications

• High school diploma or GED required.
• Bilingual English/Spanish preferred.
• Excellent verbal and written communication.
• Valid driver license and auto insurance.
• Ability to pass background checks and meet safety requirements.

Responsibilities

• Work primarily out in the community to reach patients.
• Be a patient advocate.
• Conduct standardized patient intake and risk assessments.
• Document all interactions in Pathways and Sea‑Mar’s EHR.
• Attend trainings and meetings for Pathways and Sea‑Mar.
• Coordinate with HUB partners to identify high‑risk clients.
• Provide referrals and transportation assistance to clients.
