Job summary

Qualifications

• Minimum 1 year of experience in healthcare or social services.
• Strong oral and written communication skills.
• Able to manage multiple priorities and remain positive.
• Committed to serving the community and building relationships.
• Proficient with Windows, GSuite, and Microsoft tools.
• Access to reliable transportation and ability to travel locally.
• US work authorization.

Responsibilities

• Establish and maintain relationships with patients and community partners to coordinate needs.
• Manage patient referrals defined by the care team and collaborate on action plans.
• Facilitate communication among patients, families, and providers involved in care.
• Build relationships with local community organizations for patient benefit.
• Connect patients to housing, transportation, food resources and other barriers to health.
• Assist patients with applications for eligible benefits and resources.
• Promote goal setting and progress with the care team.
• Meet patients at Oak Street Health centers, homes, or community sites.
• Spend time outside the center in patient-centered locations; have reliable transportation.
• Support care team decisions in interdisciplinary meetings and HIPAA-compliant documentation.
