Job summary

Qualifications

• Community Health Worker certification is required.
• 4–5 years in a healthcare setting or relevant experience.
• Bilingual English and Spanish required.
• Experience coordinating care and connecting patients with community resources.
• Ability to work independently and collaboratively with a multidisciplinary team.
• Familiarity with Medicare/Medicaid programs is a plus.

Responsibilities

• Provides comprehensive community health support to an assigned patient panel.
• Coordinates care with clinicians, social services, and external partners.
• Conducts home visits when needed and documents interventions.
• Educates patients on preventive care and self-management.
• Maintains knowledge of benefits like SNAP, LIS, and PAP.
