Suvida Healthcare is seeking a Clinical Pharmacist to strengthen the provider-patient relationship by enhancing medication management and program integration. You will support clinical staff, optimize therapy, and strive to reduce costs while improving outcomes. This role emphasizes patient-centered care and collaboration with an interdisciplinary team.

Ideal candidates have a Pharm D. degree, an active AZ license (or ability to obtain one), 3+ years of clinical pharmacist experience, and

Qualifications

• Pharm D. degree or equivalent education in pharmacy.
• Active, unrestricted Pharmacy license in Arizona, or the ability to obtain one.
• 3+ years of clinical pharmacist experience.
• 3+ years' experience effectively builds informal relationship networks inside and outside the organization.
• Advanced in Microsoft Office platforms including Word, Excel, and PowerPoint.
• Bilingual English and Spanish required.

Responsabilités

• Provide patient centered care to optimize medication therapy and improve outcomes.
• Perform medication management services to identify, resolve, and prevent problems.
• Coordinate medication reconciliation across the continuum of care and educate patients.
• Review patient care plans, medications, allergies, labs to resolve problems.
• Assess prescribed therapy for appropriateness, dosing, and adherence.
• Follow-up with patients/caregivers and document outcomes.
• Collaborate with providers to establish disease state goals.
• Provide drug information and participate in quality improvement initiatives.
