Job summary

Suvida Healthcare seeks a Clinical Pharmacist to support medication management, clinical program integration, and practice transformation. You will optimize therapy, reduce costs, and improve patient and provider satisfaction through collaboration with the care team.

You will review plans, address adherence, and provide patient education while ensuring seamless care across the continuum. Fluency in English and Spanish is preferred for patient interactions.

Qualifications

• 3+ years of clinical pharmacist experience
• Proven ability to build informal relationship networks inside and outside the organization
• Advanced in Microsoft Office platforms (Word, Excel, PowerPoint)

Responsibilities

• Provide patient-centered care to optimize medication therapy and improve clinical outcomes
• Perform medication management to identify, resolve, and prevent drug-related problems
• Coordinate medication reconciliation across the care continuum and monitor outcomes
• Review patient care plans, meds, allergies, labs, and medical records to resolve issues
• Communicate and collaborate with providers and care teams to set disease-state goals
• Deliver drug information and education to patients and caregivers
