Job summary

Responsibilities

• Coordinate integrated case management for adult, youth, and young adult populations.
• Engage, assess, plan, advocate, coordinate, and link clients to supports to promote wellness and stability.
• Maintain documentation and data reporting for grant compliance and program evaluation.
• Collaborate with law enforcement, schools, healthcare providers, and community partners to support client engagement and wraparound services.
