Job summary

SummitStone Health Partners is seeking a Full Time benefited Case Manager in Fort Collins, CO. The role coordinates integrated case management for adult and youth populations referred through PREVAIL, focusing on behavioral health, housing, benefits, and other supports to promote stability and community connection.

The position requires a Bachelor’s degree in behavioral health and at least one year of related experience, with trainings in GAIN, CiviCore, Motivational Interviewing, and Mental

Qualifications

• Bachelor’s degree in behavioral health required.
• One year of experience in behavioral health, case management, or related field required.
• Ability to complete required program trainings (GAIN, CiviCore, Motivational Interviewing, etc.) preferred.

Responsibilities

• Receives, reviews, and manages referrals from law enforcement, community partners, schools, families, and other sources.
• Conducts outreach, enrollment support, goal planning, and progress monitoring for participants.
• Provides trauma-informed, harm-reduction, person-centered intensive case management and service coordination.
• Documents services, outreach attempts, assessments, referrals, goals, and case management contacts in required systems.
• Participates in program coordination meetings and maintains collaboration with community partners.
