Job summary

SummitStone Health Partners in Fort Collins, CO seeks a Prevention and Harm Reduction Coordinator - Bilingual to coordinate outreach, education, and resource navigation for underserved populations. The role focuses on trauma‑informed engagement and ensuring access to behavioral health and community resources.

Responsibilities include planning outreach, providing bilingual education, and coordinating services with partners.

Qualifications

• Bilingual proficiency required, with successful completion of a language assessment.
• Bachelor’s degree in public health, social work or related field required.
• At least one year of directly relevant experience in the behavioral health field required.
• At least one year of experience in Harm Reduction service delivery required.
• Experience in coordinating services with other agencies required.

Responsibilities

• Coordinate harm reduction prevention initiatives through planning, implementation and outreach.
• Conduct community outreach and provide bilingual education on harm reduction and prevention.
• Provide trauma‑informed engagement and support to youth and adults through outreach and programming.
• Deliver mobile harm reduction outreach in rural and underserved communities.
• Assist individuals in accessing behavioral health, healthcare, housing, and community resources.
