Job summary

SummitStone Health Partners in Fort Collins, CO seeks an experienced Case Manager to coordinate integrated services for adult, youth, and young adult clients referred through PREVAIL. You will assess needs, plan goals, coordinate supports, and link participants to behavioral health, housing, and benefits, delivering trauma-informed, harm-reduction, and client-centered care.

You will work closely with law enforcement, schools, healthcare providers, and community partners to ensure engagement,

Qualifications

• Bachelor’s degree in behavioral health or related field required.
• One year of experience in behavioral health, case management, or related human services field required.

Responsibilities

• Receives, reviews, and manages referrals from law enforcement and partners; prioritizes PREVAIL referrals and outreach.
• Conducts outreach, enrollment support, goal planning, and ongoing progress monitoring.
• Provides trauma-informed, harm-reduction, and person-centered intensive case management and service coordination.
• Documents services, outreach, assessments, referrals, and case management contacts in required systems with data quality.
• Supports program fidelity and grant compliance by completing required trainings.
