Job summary

SummitStone Health Partners in Fort Collins, CO, is seeking a Case Manager to coordinate integrated services for adult, youth, and young adult populations under PREVAIL. You will engage clients, assess needs, plan goals, and link to behavioral health, housing benefits, and other supports, with a trauma-informed, harm-reduction approach.

Responsibilities include coordinating referrals, conducting outreach, maintaining documentation, and collaborating with law enforcement, schools, and community

Qualifications

• Bachelor's degree in Behavioral Health or related field required.
• One year of experience in behavioral health, substance use treatment, case management, or related field required.
• Professional proficiency in English and Spanish is required.

Responsibilities

• Coordinates integrated case management for adult, youth, and young adult populations under PREVAIL.
• Engages, assesses, plans, advocates, coordinates, and links clients to supports to promote wellness and safety.
• Maintains trauma-informed, harm-reduction, and person-centered services with outreach and documentation.
• Collaborates with law enforcement, schools, behavioral health providers, housing and social services.
