Job summary

North Bay Regional Center is seeking a Social Services Case Manager to manage NBRC clients' services and ensure IPP objectives are met through person-centered planning and resource coordination.

Applicants should have a Master’s degree (preferred), at least 3 years’ experience with developmental disabilities, and bilingual Spanish-English abilities. In-person work is required; a hybrid option may be available after 3 months of in-office performance.

Qualifications

• Master’s Degree preferred.
• 3 years’ experience working with individuals with developmental disability.
• Bilingual language ability (Spanish to English) is required.

Responsibilities

• Using person-centered planning to assist clients in use of public and private resources before using regional center funds to purchase services.
• Monitoring vendor services and verifying that services are delivered in accordance with the purchase of service authorizations
• Evaluating the effectiveness of clients’ Individual Program Plans; ensuring that appropriate community resources are made available to the client
• Conducting annual reassessments of clients’ developmental status in conjunction with the interdisciplinary team
• Conducting client assessments on an occasional basis
