Qualifications

• Bachelor's degree or higher in Behavioral or Social Science or related field.
• Read, write and speak Spanish fluently.
• At least one year experience as a supervised social worker in a health care setting.

Responsabilités

• Conduct assessments to determine clients' needs for health and human services.
• Develop individualized, comprehensive service plans.
• Coordinate resources using in-house sources, referrals, and contracted providers.
• Maintain accurate case records, summaries, and reports.
• Build supportive relationships with clients, families, and community partners.
• Communicate effectively with individuals from diverse backgrounds and abilities.
