Qualificações

• Minimum one year of experience in health or human services.
• Associate’s degree in health or human services, or a related certificate, or at least one year of relevant experience.
• Experience contributing to client service plans and assessing eligibility.

Responsabilidades

• Serve as liaison for participants, families, providers, and partners.
• Conduct assessments and develop person-centered service plans.
• Monitor service delivery and address care gaps.
• Provide casework and support during critical situations, including hospitalizations.
• Maintain accurate records compliant with HIPAA and state laws.
• Build natural support networks and promote participant engagement.
• Conduct outreach to stakeholders to expand access to services.
