Job summary

The W Group is seeking a compassionate Case Manager to join our Bronx-based assisted living team. You will coordinate care, guide admissions, and ensure residents receive the support they need, working closely with a collaborative care team.

Fluency in English and Spanish is preferred, and prior case management in long-term care helps. You will document care delivery in ALIS and advocate for residents with a warm, trust-building approach.

Qualifications

• At least 1 year of case management experience.
• Fluency in English and Spanish preferred.
• Experience with ALIS or similar EMR system is a plus.
• Strong communication with residents, families, and providers.
• Highly organized with ability to manage multiple priorities.

Responsibilities

• Serve as main point of contact between residents, families, physicians, and outside care providers.
• Conduct resident assessments and coordinate admissions and re-admissions from the community, hospitals, and nursing homes.
• Coordinate care across the continuum with nursing and interdisciplinary team.
• Advocate for residents and uphold Residents’ Bill of Rights.
• Maintain accurate documentation in ALIS in line with standards.
