Job summary

Hebrew SeniorLife is seeking a Nurse Care Manager to support residents at an on-site affordable housing community in Chelsea, MA. The role focuses on holistic case management, wellness assessments, care coordination, and connecting residents to needed services, with emphasis on Spanish-speaking residents.

Minimum qualifications include RN with 1 year experience or LPN with 3 years, strong triage and communication skills, and fluency in Spanish.

Qualifications

• RN with 1 year of experience or LPN with 3 years of experience required.
• Experience in aging services preferred, home health and dementia care a plus.
• Excellent triage and critical thinking skills are required, as well as compassion for working with a senior population.

Responsibilities

• Partner with wellness coordinator and resident services team to provide comprehensive case management.
• Provide regular preventative outreach to residents to assess needs and health.
• Conduct wellness assessments and follow up on identified needs with referrals and services.
• Coordinate with PCPs, specialists, hospitals, mental health, and community providers.
• Active follow-up on hospitalizations, rehab stays, and ER visits for safe discharges and ongoing services.
• Track residents with special needs and refer as appropriate.
• Document all work electronically in online software.
