The W Group is seeking a compassionate Case Manager for Bronxwood Assisted Living Facility in the Bronx, NY. You will coordinate admissions, manage care plans, and support residents and families with a resident-centered approach.

Fluency in English and Spanish is preferred, with experience in ALIS or similar EMR systems a plus. The role emphasizes organization, communication, and teamwork in a supportive environment.

Qualifications

• At least 1 year of case management experience (required).
• Long-term care or assisted living experience is a strong plus.
• Fluency in English and Spanish (strongly preferred).
• Experience with ALIS or similar EMR system is a plus.
• A clear, warm communicator who builds trust with residents, families, and outside providers.
• Highly organized and able to manage multiple priorities.
• A genuine passion for working with seniors and a team-first mindset.

Responsabilités

• Serve as the main point of contact between residents, families, physicians, and outside care providers.
• Conduct resident assessments and coordinate admissions and re-admissions from the community, hospitals, and nursing homes.
• Coordinate care across the continuum, working closely with nursing and the interdisciplinary team.
• Advocate for residents and uphold the Residents’ Bill of Rights in every interaction.
• Maintain accurate, timely documentation in ALIS in line with facility and regulatory standards.

Bronxwood Assisted Living Facility is looking for a compassionate, organized Case Manager to join our team. In this role, you’ll be the person residents and families rely on: coordinating care, guiding admissions, and making sure every resident receives the support they need. You’ll work alongside a collaborative team of healthcare professionals who truly care about the people we serve.
