Job summary

The Providence Center is seeking a Case Manager bilingual for ACT II Prairie Ave team to support clients in reducing psychiatric and substance use symptoms and promoting independence.

You will provide person-centered, recovery-oriented case management within a multidisciplinary team, including in-person and community-based services, while coordinating housing, healthcare, education, employment, and other community resources.

Qualifications

• Associate's Degree required; Bachelor's Degree preferred in Human Services or related field.
• Must have valid driver's license and insured auto.
• Bilingual preferred; capable of bilingual communication is a plus.

Responsibilities

• Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
• Conduct comprehensive assessments to identify housing, education, employment, healthcare and community resource needs.
• Develop, present and implement individualized care plans in collaboration with clients and care teams.
• Provide ongoing monitoring, crisis intervention, and follow-up to ensure needs and progress are met.
• Maintain accurate documentation and records; prepare reports for grants, audits and program evaluation.
