Job summary

Bluebonnet Trails Community Services seeks a Case Manager to provide recovery-oriented case management under the supervision of the Clinical Program Manager. You will conduct psychosocial assessments, develop person-centered recovery plans, connect individuals with community resources, and document in the electronic health record.

The role emphasizes direct service (at least 55%), collaboration with a multidisciplinary team, crisis intervention when needed, and advocacy to promote independence

Qualifications

• Bachelor's degree in Human Services (or related field) required.
• If degree is not in Human Services, must have completed at least 24 credit hours in a human services-related field.
• Strong understanding of mental health diagnoses and treatment.
• Ability to work independently and collaboratively in a fast-paced, recovery-oriented environment.
• Must have a valid Texas driver's license and maintain approved driver status.

Responsibilities

• Conduct routine psychosocial and emotional needs assessments.
• Develop Person-Centered Recovery Plans.
• Connect youth, adults and families with community resources and rehabilitative services.
• Document services and outcomes in the electronic health record; ensure accuracy.
• Provide crisis intervention when necessary and advocate for individuals served.
