Job summary

Bluebonnet Trails Community MHMR Center is seeking a dedicated Case Manager in Round Rock, TX to provide recovery‑oriented case management under supervision. You will perform psychosocial assessments, develop Person‑Centered Recovery Plans, and ensure detailed documentation in the electronic health record.

The role requires a Bachelor’s degree in Human Services or related field, a valid Texas driver’s license, and the ability to work both independently and within a multidisciplinary team.

Qualifications

• Bachelor’s degree in Human Services or a related field is required.
• Master’s degree in Human Services or related field is preferred.
• A valid Texas driver’s license is required and must be maintained.
• If the degree is not in Human Services, 24 credit hours in a human services field are required.

Responsibilities

• Conduct routine psychosocial and emotional needs assessments.
• Develop Person-Centered Recovery Plans.
• Connect youth, adults, and families with community resources and rehabilitative services.
• Initiate crisis intervention when necessary and document all services in the electronic health record.
• Collaborate with a multidisciplinary recovery team to coordinate care and advocacy for individuals served.
