Job summary

Qualifications

• Bachelor’s Degree Required.

Responsibilities

• Assess clients' needs and eligibility for in-home services.
• Conduct intakes and provide accurate information and referrals.
• Advocate on behalf of clients and arrange for provision of services.
• Maintain ongoing communication with client and family members.
• Facilitate and assist at social, recreational, and other group activities.
