Job summary

Responsibilities

• Collaborate with local agencies to achieve client goals and build partnerships.
• Develop and implement programs and workshops based on community needs.
• Document encounters thoroughly in case notes and maintain records in a timely manner.
• Educate clients on services, housing rights, and application procedures.
• Strengthen informal support networks and coordinate with families and providers.
