SAMU Foundation is seeking a Bilingual Case Manager to work in Washington, DC, delivering trauma-informed, client-centered services to youth at the Drop-In Center. The role emphasizes coordinated supports across housing, health, and employment services.

Responsibilities include intakes, service planning, public benefits enrollment, and ongoing case management, with collaboration across multidisciplinary teams. A Bachelor’s degree and experience in social services are required.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Counseling, or related social services, science, or healthcare field.
• Experience providing case management services (two years).
• Experience with behavioral health or substance use backgrounds is preferred.

Responsabilités

• The Bilingual Case Manager provides individualized support to youth at the Drop-In Center.
• Conduct client intakes, needs assessments, and service planning.
• Assist clients with enrollment in public benefits (e.g., SNAP, Social Security, Medicaid).
• Coordinate care with behavioral health, healthcare, and housing partners.
• Maintain confidential case records in line with DHS, HMIS, and SAMU standards.
• Participate in team meetings and service coordination efforts.
• Provide crisis intervention and trauma-informed support as needed.
• Support life skills development and independent living resources.
• Engage clients to promote empowerment and self-sufficiency.
• Participate in CAHP assessments and placements as required.
