Job summary

Xiente is hiring a bilingual Case Manager to provide direct support to individuals and families in the Intensive Prevention Services (IPS) program at the Teen Center in Philadelphia. You will conduct intakes, develop service plans, connect participants with community resources, and advocate to help them achieve stability and self-sufficiency.

This full-time, onsite role requires English and Spanish proficiency, strong communication, organization, and the ability to manage a caseload of up to 17

Qualifications

• Bachelor's degree in social work, criminal justice, sociology, human services, counseling, psychology, or related field.
• At least 1 year in case management or related social services field.
• Bilingual English and Spanish required; strong communication skills.

Responsibilities

• Conduct intakes and comprehensive needs assessments.
• Create and maintain individualized service plans.
• Connect participants with community resources and services.
• Coordinate referrals and follow up on support provided.
• Monitor progress and provide ongoing case management.
• Maintain accurate case notes and documentation.
• Advocate for participants and help navigate barriers.
