Job summary

Qualifications

• Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Public Administration, Education, Sociology, Family Services, Workforce Development, or related field.
• Fluency in both English and Spanish is required.
• At least two years of case management or related human services experience.
• Experience with diverse populations; knowledge of community resources.
• Strong organizational, documentation, and time-management skills; MS Office and electronic case management systems.

Responsibilities

• Conduct participant intake, assessments, eligibility determinations, and enrollment activities.
• Develop individualized service plans identifying goals, barriers, strengths, and action steps.
• Provide ongoing case management, coaching, and advocacy throughout program participation.
• Connect participants with employment, education, workforce development, and financial wellness resources.
• Document progress and ensure compliance with agency policies and program guidelines.
• Engage with employers and community partners to expand opportunities for participants.
