Job summary

PHMC’s Bridge Intensive Prevention Services (BIPS) seeks a Case Manager to provide person-centered, trauma-informed support to youths aged 10–19 and their families in West Philadelphia. You will coordinate intake, conduct home visits, obtain school records, and advocate for youth at court hearings, while maintaining program records.

Reporting to the BIPS Program Director, you will partner with a Therapist/Counselor, support life skills groups, coordinate services with schools and communities,

Qualifications

• Bachelor’s degree required in social work, psychology, counseling or related field.
• Two years of case management experience preferred.
• Experience with trauma-informed care and at-risk youth and families.

Responsibilities

• Maintain a caseload of approximately 17 adolescents and their families with weekly family-focused case management and service coordination.
• Collaborate with Therapist/Counselor to provide trauma-informed services across program, school, community and home settings.
• Schedule intake appointments and conduct at least two home visits per month per case.
• Obtain and review school records monthly; accompany youth to court hearings and advocate for needed services.
• Submit monthly updates to DHS, DA’s Office, and Philadelphia Police Department as required.
• Facilitate life skills groups and educational sessions; educate on harm reduction strategies and wellness.
