Job summary

Sea Mar Community Health Centers in Vancouver, WA, seeks a Behavioral Health Specialist for full-time community-based case management. You will support mentally ill, homeless, and substance-use clients and coordinate services with a multidisciplinary team to improve outcomes.

Applicants need an Associate’s degree in Behavioral Health or related field, two years of relevant experience, eligibility for State Counselor Registration, and a valid WA driver’s license.

Qualifications

• Associate’s degree in Behavioral Health, Health Sciences, or related field.
• Two years of closely related work experience is required.
• Eligible for Agency Affiliated Counselor Registration from the State Department of Health.

Responsibilities

• Provide care appropriate to the age of patients served.
• Conduct intake screenings to determine client’s need for case management.
• Develop Client Service Plans and coordinate referrals to services.
• Maintain regular contact with clients and support adherence to the Service Plan.
• Document activities and participate in interdisciplinary team reviews.
