Services for the UnderServed is seeking a dedicated Case Manager in Brooklyn, NY. This role focuses on providing comprehensive case management within a Certified Community Behavioral Health Clinic (CCBHC) to serve vulnerable populations with complex needs.
Field work dominates the schedule, with travel required and occasional crisis response. Responsibilities include developing service plans, linking clients to medical, behavioral health, and social services, and advocating for client needs

Max: USD $55,000.00/Yr.
Case Manager ensures the best quality of care and services for program consumers in a Certified Community Behavioral Health Clinic (CCBHC). "CCBHCs are designed to provide a comprehensive range of mental health and substance use disorder services, particularly to vulnerable populations with the most complex needs"\.
The ideal candidate will provide Case Manager services to clients within the scope of the clinic and be creative, innovative, open to change.
