Legacy Community Health is seeking a Bilingual Pediatrics Care Team Assistant to support patient communications, medication management, and care coordination in a busy pediatric setting.

The role supports providers and the care team, ensuring smooth pre-visit planning, timely lab result handling, and accurate documentation within the EHRS system. Tulsa? (Note: this is a pediatric-focused on-site position in Houston, TX.)

Qualifications

• High-school level education or equivalency.

Responsibilities

• Serve as a welcoming and dependable resource for patients, families, caregivers, providers, and community partners.
• Identify medically vulnerable patients who may need additional services, monitoring, or referrals.
• Support quality and compassionate care through effective coordination, timely communication, and accurate documentation.
• Collaborate with providers, nursing teams, referral staff, front-desk colleagues, and community resources.
• Contribute to a safe, respectful, and patient-centered clinic environment.
• Build experience across care coordination, clinical workflows, patient support, and broader healthcare ecosystem.
• Assist with transitions of care, including obtaining discharge records and specialty notes.
• Manage and maintain the site’s Care Coordination Desktop.
• Assist patients with entry to ancillary support services, including prescription assistance and social work.
• Assist with medication refills and prior authorizations as directed by providers.
