Qualifications

• Bachelor's degree or higher in a relevant field.
• 1–3 years of support coordination experience with NJ DDD.
• Knowledge of NJCAT and Person-Centered Planning Tool.

Responsibilities

• Assess needs and develop individualized service plans.
• Coordinate services with providers and teams.
• Monitor progress and adjust plans as needed.
• Maintain compliant documentation and records.
• Communicate effectively with individuals, families, and providers.
