Job summary

Owen Health Care in New Jersey is seeking a bilingual Support Coordinator to help individuals with developmental disabilities access services and resources, develop individualized service plans, and coordinate approved supports. You will work closely with families, providers, and an interdisciplinary team to ensure quality, compliant, and person-centered care.

The role requires strong communication, documentation, and advocacy skills, with potential travel to client homes and program locations

Qualifications

• Bachelor's degree in public health, social services, psychology, human services, or related field required.
• Master's degree preferred for advanced practice and leadership.
• Bilingual communication skills required for effective collaboration with diverse populations.

Responsibilities

• Assess and identify client needs; develop Person-Centered Plans.
• Coordinate services using NJCAT to determine goals and resources.
• Develop, update, and monitor New Jersey ISP plans.
• Facilitate team meetings at various program locations.
• Maintain compliant, timely documentation and records.
• Advocate for individuals and families with providers and regulatory bodies.
