Bilingual Support Coordinator

Position Summary

The Bilingual Support Coordinator works closely with individuals, families, providers, and interdisciplinary team members to identify support needs, develop individualized service plans, coordinate approved services, and monitor the quality and effectiveness of those services.

The successful candidate will provide responsive, person-centered case management while maintaining accurate documentation and compliance with New Jersey Division of Developmental Disabilities and Medicaid requirements.

Essential Job Functions
  • Assess individual needs and complete the Person-Centered Planning Tool.
  • Use NJCAT information to help determine support needs, goals, service eligibility, and appropriate resources.
  • Develop, update, and monitor New Jersey Individualized Service Plans.
  • Coordinate and facilitate team meetings at residential programs, day programs, client homes, and other community locations.
  • Address personal, medical, nutritional, health, safety, behavioral, employment, and community-integration needs within each individualized plan.
  • Connect individuals and families with qualified providers, community programs, and other appropriate resources.
  • Maintain consistent communication with individuals, families, providers, and members of the support team.
  • Monitor service delivery, individual progress, changing needs, and achievement of identified goals.
  • Maintain timely and accurate electronic records, case notes, service plans, and supporting documentation.
  • Ensure compliance with New Jersey DDD, Medicaid, company, and applicable regulatory requirements.
  • Participate in required training, supervision, quality reviews, and performance-development activities.
Qualifications
  • Bachelor's degree in public health, social services, psychology, human services, or a related field; master's degree preferred.
  • At least one to three years of support coordination experience working with individuals served by the New Jersey Division of Developmental Disabilities.
  • Bilingual communication skills required.
  • Knowledge of the NJCAT, Person-Centered Planning Tool, NJISP process, Medicaid requirements, and New Jersey DDD service systems.
  • Strong written and verbal communication skills.
  • Ability to facilitate meetings and clearly present information to individuals, families, providers, and interdisciplinary teams.
  • Excellent organization, case-management, documentation, and time-management skills.
  • Ability to manage competing priorities and meet required deadlines.
  • Professional, compassionate, and customer-focused approach.
  • Ability to build collaborative relationships and effectively advocate for individuals and families.
  • Valid driver's license, reliable personal vehicle, and ability to travel to client homes, programs, meetings, and other service locations.
  • Completion of all required New Jersey DDD/CDS training-or the ability to complete the training within the required timeframe.
