Job summary

Paycom - ATS in New York State is seeking a Care Manager to coordinate services for individuals with intellectual and developmental disabilities, advocate for member-centered Life Plans, and connect families with needed supports.

In a hybrid role, you will meet members in their homes and communities, work with an interdisciplinary team, document accordingly, and travel locally within the service area. A valid driver’s license and NY residency within 100 miles of the office are required.

Qualifications

• Bachelor’s degree with 2 years of relevant experience, or Master’s degree with 1 year, or RN with 2 years.
• Experience in human services, I/DD, mental health, or related field preferred.
• Strong communication, problem-solving, and organizational skills.
• Ability to work independently in a fast-paced, field-based role.

Responsibilities

• Develop and manage Life Plans based on comprehensive assessments.
• Coordinate medical, behavioral health, and community-based services.
• Advocate for members to access services and supports.
• Lead interdisciplinary team meetings and collaborate with providers.
• Maintain accurate documentation and ensure billing readiness.
