Job summary

The Children'S Clinic in Bellflower, CA is hiring a Care Coordinator for a Regular Full-Time role. This position coordinates patient referrals, schedules provider appointments, and acts as liaison among the care team to ensure appropriate patient care.

Requirements include 2-4 years of medical office experience in outpatient settings; bilingual skills in Spanish or Khmer are strongly preferred; a high school diploma or GED is required and Medical Assistant certification is a plus.

Qualifications

• High School Diploma or GED required.
• Medical Assistant Certification is a plus.
• Must have 2-4 years of medical office experience in an outpatient clinical setting.

Responsibilities

• Completes routine reports, messages, and forms.
• Communicates in written and verbal form, using correct grammar.
• Submits all correspondence within the requested time frame.
• Relays all messages as instructed.
• Organizes and files records according to established clinic procedures.
• Files records in chronological and alphabetical order.
• Effectively utilizes office machines including computer, fax, and photocopier.
• Collects and distributes mail in a timely manner per clinic procedures.
• Participates in huddles.
• Takes messages for the Care Team.
• Follows all policies and procedures.
• Sends authorizations and referrals, follows up on referrals to outside agencies to ensure continuity of patient care.
