A&B Homecare Solutions in Stamford, CT is seeking a Bilingual Care Coordinator Associate to ensure patient care needs are met efficiently. You will coordinate schedules, communicate with patients and families, and document visits in our system. This role requires a high school diploma, at least one year in customer service, and bilingual English/Spanish fluency.

Home healthcare experience is preferred. On-site work with regular daytime hours and rotating on-call shifts provide meaningful patient

Qualifications

• High school diploma or equivalent required.
• 1+ years of customer service experience required.
• 1+ years of home healthcare experience preferred.

Responsibilities

• Assign field staff schedules and confirm assignments with employees and patients.
• Speak with patients or families regarding schedules, call-offs, cancellations, and appointments.
• Document client data and visits in the scheduling system.
• Backup to Intake department for home care service requests.
• Participate in on-call rotations to provide after-hours support.
• Communicate effectively within the office and with clients' care teams.
