Care Finders Total Care is seeking a Care Coordinator Associate to ensure patient care needs are met efficiently. This role requires handling referrals, coordinating intake, and serving as the primary contact for patients and providers.

The position offers pay of $20–$24 per hour, Monday to Friday, 8:30 AM–5:00 PM, in Toms River, NJ, with English and Spanish language proficiency required to support diverse patient populations.

Qualificações

• 1+ years of customer service experience.
• Bilingual in English & Spanish required.

Responsabilidades

• Receive and process referrals from hospitals, physicians, and other providers.
• Collaborate with internal teams to determine patient eligibility and start intake.
• Serve as the primary contact for patients, families, and healthcare professionals regarding care coordination.
• Address inquiries, concerns, and provide guidance on accessing services and resources.
• Schedule patient appointments, coordinating availability of healthcare professionals and patient preferences.
• Monitor and update schedules to accommodate changes, cancellations, and rescheduling needs.
• Communicate pertinent patient information, including diagnosis, treatment plans, and care instructions, to the appropriate individuals.
• Maintain accurate and up-to-date electronic and physical records of patient information, appointments, and communication.
• Provide compassionate and empathetic support to patients and their families throughout the care coordination process.
