Job summary

Care Finders Total Care, part of a Modivcare Personal Care Service, seeks a Care Coordinator Associate. You will help ensure patient care needs are met efficiently and compassionately.

The role offers an hourly pay between $20 and $24, a Monday–Friday 8:30 AM–5:00 PM schedule, and an office in Toms River, NJ. English & Spanish language skills are required to serve diverse patients and families.

Qualifications

• High school diploma or equivalent.
• 1+ years of customer service experience.
• 1+ years of home healthcare experience preferred.
• Bilingual in English & Spanish required.

Responsibilities

• Receive and process referrals from hospitals, physicians, and other healthcare providers.
• Collaborate with internal teams to determine patient eligibility and initiate intake.
• Serve as the primary point of contact for patients, families, and healthcare professionals regarding care coordination.
• Address inquiries and provide guidance on accessing services and resources.
• Schedule patient appointments and coordinate availability of healthcare professionals.
• Monitor and update schedules for changes, cancellations, and rescheduling.
• Communicate patient information, including diagnosis and treatment plans, to the appropriate individuals.
• Maintain accurate records of patient information, appointments, and communications.
• Provide compassionate support to patients and families throughout the care coordination process.

You will...
