Qualifications

Responsibilities

• Oversees chronic care management and transitions of care for high-risk patients.
• Coordinates with providers, payors, and care teams to ensure timely, cost-effective care.
• Performs assessments and documents in the EMR with attention to social determinants.
• Supports discharge planning and post-acute transitions within the care continuum.
• Engages in performance improvement activities and data-driven care.
