Job summary

211 LA is seeking a Temporary Care Coordinator Specialist to deliver critical care coordination for individuals directed to specialized programs in Pasadena, CA. You will evaluate client needs, refer to services, and coordinate with partner organizations while maintaining records in CareSuite.

The role requires Spanish bilingual proficiency, strong organizational skills, and flexibility to respond to emergencies with an undetermined schedule.

Qualifications

• Experience with Care Coordination or Case Management.
• Ability to communicate with diverse backgrounds.
• Strong organizational and multitasking abilities.
• Bilingual Spanish proficiency.

Responsibilities

• Assess client needs and safety planning.
• Provide referrals and advocacy for clients as needed.
• Coordinate care with partner agencies and prevent duplication of services.
• Follow up to ensure services accessed.
• Document interactions in CareSuite.
• Serve as liaison between 211LA County and partners.
• Travel as needed; evenings/weekends/holidays may be required.
• Respond to county emergencies with flexible schedule.
